Ethmoidal Sinus Mucocele as a Cause of Acquired Brown Syndrome

چکیده
lenge. Surgery can lead to iridodialysis, cyclodialysis, and subsequent bleeding [5]. The following steps prevented these complications in the two patients presented here. First, we used a modified Siepser technique for creation of the knot in situ. This type of knot minimizes the force pulling on the iris from the direction of the corneolimbal angle. Second, we used an IOL manipulator to create a counter-force when the needle was passed through the iris. Third, the viscoelastics is inserted in a step-wise manner. This was performed to prevent iris prolapse through the primary incision, which would subsequently trigger a sharp spike in IOP. Finally, the needle was maneuvered extremely gently when passing horizontally through the anterior chamber to as to avoid contact with the lens. This is the first report to describe the adaptation of a modified Siepser slipknot technique to treat symptomatic LPI in phakic eyes.

Spontaneous drainage of an ethmoidal mucocele: a possible cause of pneumosinus dilatans.
Idiopathic expansion of a paranasal sinus containing air and without an associated mass has been termed pneumosinus dilatans [1] . This rare disorder has been known to occur in all the paranasal sinuses, but it most commonly affects the frontal sinus. Clinical symptoms include pain , visual loss, proptosis, galactorrhea, and hypopituitarism [2-4]. Proposed mechanisms of origin include developme...
متن کاملBlinding Orbital Apex Syndrome due to Onodi Cell Mucocele
The onodi cell is a posterior ethmoidal cell that is pneumatized laterally or superiorly to the sphenoid sinus with close proximity to the optic nerve. A mucocele, a benign, expansile, cyst-like lesion of the paranasal sinuses, may uncommonly involve the onodi cell causing compression of the optic nerve and nearby structures. In this paper, we report a rare case of onodi cell mucocele causing o...
متن کاملEthmoidal Mucocele Presenting as Oculomotor Nerve Paralysis
A 56-year-old male was admitted with an acute headache and sudden ptosis on the right side. No ophthalmological or neurological etiologies were apparent. A mucocele of the right posterior ethmoid sinus was observed with radiology. After the marsupialization of the mucocele via a transnasal endoscopic approach, the patient's symptoms (oculomotor nerve paralysis and headache) resolved in 4 weeks....
متن کاملFronto-ethmoidal mucocele as a cause of unilateral proptosis.
MUCOCOELE of the frontal or ethmoidal sinuses is an uncommon cause of unilateral proptosis which may produce considerable difficulty in diagnosis. There seems a natural tendency for these cases to be sent to an eye specialist, and a curious reluctance for him to refer the patient to an E.N.T. surgeon, often preferring a neurological opinion which may involve the patient in expensive and unpleas...
متن کاملEfficacy of endoscopic sinus surgery for paranasal sinus mucocele including modified endoscopic Lothrop procedure for frontal sinus mucocele.
This study evaluated the efficacy of the modified endoscopic Lothrop procedure (MELP) for complicated frontal mucoceles and endoscopic marsupialization for other paranasal sinus mucoceles. It was a retrospective, consecutive case review of sinus mucoceles treated endoscopically by a single surgeon over a four-year period (1998-2002). There were 41 mucoceles in 28 patients, including 24 frontal,...
